If you are 18 years of age or older please complete the Adult Intake Form.
If you are under 18 years of age please have a parent or guardian complete the Child Intake Form.
Please complete the forms and either mail or bring them into your first visit.
PLEASE NOTE: Arrive 10 minutes early, because you will have to sign Consent to Treatment and our Health Insurance Portability and Accountability Act (HIPAA) paperwork.
If you are filing insurance we will have to make a copy of your insurance card, so be sure to bring your card with you.If you are UNDER 18 years of age, a parent or guardian MUST attend the first session with you to sign the paperwork.
Appointments:
Clients are seen by appointment only. Appointments are scheduled Monday through Saturday. Evening hours are Monday through Thursday.
The business office is open 8:30 am to 5:00 pm Monday through Friday to answer your questions or to help schedule an appointment.

Fees:
In order to hold the line on the cost of therapy while maintaining a superior level of professional care, payment is expected in full, or the insurance co-pay, on the day treatment is rendered.
